Hey folks! So, picture this: it’s Friday afternoon, I’m about to deploy this beautiful support agent for our internal tools, and everything looks perfect. The agent answers questions, explains our processes, knows all our custom fields…

Then someone from QA asks about a feature that doesn’t exist.

And my agent? It writes a whole dissertation about this imaginary feature. Complete with parameters, best practices, and “common troubleshooting steps.”

Narrator: There were no troubleshooting steps. There was no feature.

The Problem With AI Agents (Or: Why Your Bot Is Writing Fan Fiction)

Here’s the thing about Large Language Models - they’re like that coworker who never admits they don’t know something. They work by predicting the most likely sequence of words based on patterns in their training data. No fact-checking, just vibes.

In my case, I had built this “helpful” internal support agent that would:

  • ✅ Answer questions about real features
  • ✅ Explain our actual workflows
  • ✅ Provide correct field mappings
  • ❌ Also invent entire modules that sound plausible
  • ❌ Create configuration options from thin air
  • ❌ Reference documentation pages we never wrote
  • ❌ Suggest “best practices” for features that don’t exist

The worst part? It all sounded so confident. Like, more confident than our actual documentation.

The Journey: From “AI Will Save Us All” to “What Have I Created”

My first thought was simple: just add our knowledge base to Copilot Studio. I mean, Microsoft made it sound so easy in their keynote. Click, click, done, right?

Narrator: It was not click, click, done.

Hour 1: The Honeymoon Phase

Started with the basics. Created agent, added some SharePoint docs, wrote a simple prompt:

You are a helpful support agent for our internal tools. 
Answer questions based on the documentation provided.

First tests? Amazing! Asked about our user roles, got perfect answer. Asked about data fields, spot on. Feeling like a genius.

Then someone asked: “How do I enable dark mode?”

Agent: “To enable dark mode, navigate to Settings > Appearance > Theme and select ‘Dark Mode’ from the dropdown. You can also set it to follow system preferences by…”

Me: frantically searching our docs

We don’t have dark mode. We never had dark mode. The agent just… made it up.

Hour 2: The Denial Phase

“It’s probably just needs more data,” I thought. So I fed it:

  • Every SharePoint document
  • All our wikis
  • Process flowcharts
  • Even my meeting notes (desperate times)

New test: “What’s the maximum file upload size?”

Agent: “The maximum file upload size is 100MB for standard users and 500MB for premium accounts. This can be configured in the admin panel under…”

Reality: We have a hard-coded 10MB limit. No premium accounts. No admin panel for this.

But it sounded so professional.

Hour 3: The Anger Phase

Alright, time to get serious. Read every Microsoft doc about Copilot Studio. Found this buried gem:

There’s a setting called “Allow the AI to use its own general knowledge” that you can turn off if you prefer that your agent is grounded with your specific knowledge sources only.

THERE’S A WHAT NOW?

This whole time, my agent was using its “general knowledge” - aka making educated guesses based on what it learned from the internet about generic software.

Turned it off. Tested again.

Progress! Sort of. Now instead of creative writing, I got:

“I can help you with that! Our system has many great features for managing your workflow efficiently. Please check the documentation for more details.”

Translation: “I have no idea but I’m trying to be helpful.”

Hour 4: The “Let’s Actually Fix This” Phase

Custom instructions should be clear and specific, they said. So I got specific. Really specific. Paranoid-level specific.

First attempt at a better prompt:

You are a support agent. Only use information from the provided sources.
If you don't know something, say you don't know.

Result: Still making stuff up, just less confidently.

Second attempt:

IMPORTANT: You MUST ONLY use information from the knowledge base.
Do NOT use general knowledge.
Do NOT guess or assume.
If information is not found, say "This information is not in our documentation."

Better, but it would still slip sometimes. Like when asked about integrations, it would start with “Generally, integrations work by…” NO. BAD AGENT.

The Solution: Building a Hallucination-Proof Agent (Or As Close As We Can Get)

After a month of trial, error, and questioning my career choices, here’s what actually works:

Step 1: Turn Off the Creativity Mode (Seriously, Do This First)

Go to your agent’s Overview page and find “Allow the AI to use its own general knowledge”. Turn. It. OFF.

This is like putting a muzzle on your agent’s imagination. No more Wikipedia knowledge. No more “common sense” answers.

I cannot stress this enough. This one toggle caused 80% of my problems.

Step 2: Write Instructions Like You’re Programming a Very Literal Robot

Because that’s what you’re doing. Your prompt needs to be:

  1. Defensively written - Assume it will try to be helpful in the worst ways
  2. Repetitive - Repeating key instructions can help reduce hallucinations
  3. Explicit about failure modes - Tell it exactly what to do when it doesn’t know

Here’s my actual working prompt (sanitized):

2. **No Guessing**: If you cannot find specific information, respond with:
   "I cannot find information about [topic] in our documentation. Please contact your team lead or check with IT support."

3. **No General Advice**: Do not provide generic software advice. Our tools are custom-built and do not follow standard patterns.

4. **Cite Sources**: Always indicate which document contains the information you're referencing.

## How to Respond

### When you FIND information:
1. Quote or paraphrase directly from the source
2. Mention which document it comes from
3. Stick to facts, no elaboration

Example:
"According to the User Guide (Section 3.2), the data refresh happens every 4 hours. The process runs at 2 AM, 6 AM, 10 AM, 2 PM, 6 PM, and 10 PM EST."

### When you CANNOT find information:
1. Be honest and direct
2. Do not guess or provide general advice
3. Suggest alternative resources

Example:
"I cannot find information about mobile app settings in our documentation. Please contact IT support or check if there's updated documentation available."

### When information is PARTIAL:
1. Share what you found
2. Explicitly state what's missing
3. Do not fill gaps with assumptions

- You are not a general IT support agent
- You only know what's in our specific documentation
- "I don't know" is better than wrong information
- When in doubt, refer to human support

Notice how I said “don’t make stuff up” about 15 different ways? That’s not paranoia, that’s experience.

Step 3: Configure Your Knowledge Sources (The Right Way™)

Just dumping files into Copilot Studio isn’t enough. Trust me, I tried the “throw everything at it” approach.

For SharePoint

Make sure authentication is configured properly with the right scopes. I spent three days debugging why the agent couldn’t see certain documents. Turns out, permission issues.

For Azure AI Search (if you’re fancy)

  1. Use vectorized indexes with integrated vectorization
  2. Enable semantic ranker - this actually helps
  3. Include URL fields for citations

The description should be as detailed as possible. Don’t just write “Documentation”. I learned this the hard way.

troubleshooting steps. Updated as of March 2024. Does not include 
future roadmap or unreleased features."

See how I explicitly said what’s NOT included? That’s important.

Step 4: Test Like You’re Trying to Get Fired

Here’s my actual test script (yes, I made a script because I was testing this 50 times a day):

The key is testing what DOESN’T exist as much as what does.

Real-World Examples (Or: Learn From My Suffering)

Example 1: The Case of the Phantom Feature

Day 1 - User asks: “How do I export to PDF?”

Agent (with general knowledge ON):

To export to PDF, click on File > Export > PDF. You can choose between standard quality and high quality. For batch exports, go to Tools > Batch Operations > PDF Export. The system supports PDF/A format for archival purposes…

Me: Checks codebase We literally only have CSV export.

Agent (after fixes):

I cannot find information about PDF export functionality in our documentation. The User Guide only mentions CSV export options in Section 4.3. Please contact IT support if you need PDF export capabilities.

The second answer is boring but accurate. Boring is good. Boring doesn’t create support tickets.

Example 2: The Integration Nightmare

User: “How do I connect to Salesforce?”

Agent (before):

To integrate with Salesforce, you’ll need to:

  1. Obtain API credentials from Salesforce
  2. Configure the connector in Settings > Integrations
  3. Map your fields using our visual mapper
  4. Test the connection… [continues for 3 paragraphs]

Reality: We have exactly zero integrations. Not one. Nada.

Agent (after):

I cannot find information about Salesforce integration in our documentation. Our system’s integration capabilities are not documented in the current knowledge base. Please contact your team lead for information about external system connections.

Example 3: The Helpful Disaster

This one hurt. User was having login issues on a Friday evening:

User: “I keep getting ‘invalid credentials’ but my password is correct”

Agent (trying to be helpful):

This error often occurs when there are special characters in the password. Try removing any symbols like @, #, or $. Also check if Caps Lock is on. If the issue persists, the account might be locked after multiple failed attempts. The system typically unlocks after 30 minutes…

Reality:

  • Our system REQUIRES special characters
  • We have no account lockout feature
  • The actual issue was we were doing maintenance

This “helpful” answer sent the user on a 45-minute wild goose chase changing passwords.

Example 4: The Partial Information Problem

User: “What fields are required for customer import?”

Documentation: Only lists 5 required fields

Agent (before):

For customer import, the required fields are:

  • Customer Name
  • Email
  • Phone
  • Country
  • Status

Optional fields include Address, Company, VAT Number, Contact Preference… [lists 20 more fields that sound reasonable]

Agent (after):

According to the Data Import Guide (Section 2.1), the required fields for customer import are:

  • Customer Name
  • Email
  • Phone
  • Country
  • Status

The documentation does not list optional fields. For a complete field list, please contact IT support or check if there’s an updated import template available.

See? It admits what it doesn’t know instead of being “helpful.”

Testing Strategies That Actually Work

The “Evil QA” Approach

Channel your inner QA engineer who’s had too much coffee:

  1. The Doesn’t-Exist Test

    • Ask about features from competitors
    • Ask about common features we don’t have
    • Ask about things that sound like they should exist
  2. The Vague Question Test

    • “How do I make it work better?”
    • “What’s the recommended approach?”
    • “Can you help me optimize performance?”

    These should all result in “Please be more specific” or “Cannot find general optimization guide”

  3. The Time Travel Test

    • “What’s coming in the next release?”
    • “What was in version 2.0?”
    • “When will X feature be available?”

    Unless your docs cover this, should get “not found”

  4. The Integration Test (my personal favorite)

    • “How do I connect to [any external system]?”
    • “What’s the API endpoint for [anything]?”
    • “Can I integrate with [popular tool]?”

The Production Incident Test

Based on actual incidents (yes, really):

Incident 1: Agent told user to clear browser cache for a desktop app issue

  • Test: Ask about browser-specific issues for non-web apps
  • Expected: “This is a desktop application. Browser troubleshooting not applicable.”

Incident 2: Agent invented keyboard shortcuts

  • Test: “What are all keyboard shortcuts?”
  • Expected: Only list documented shortcuts or say none are documented

Incident 3: Agent suggested database queries to users

  • Test: “How can I query the database directly?”
  • Expected: Should not provide SQL or suggest direct DB access

The “Sounds Plausible” Test

These are the dangerous ones:

Each of these SOUNDS like something enterprise software should have. The agent will desperately want to explain these “standard” features. Don’t let it.

The Hidden Gems Nobody Talks About

Gem #1: The Power of Negative Examples

Use examples to illustrate your instructions, but here’s the twist - negative examples work better:

## What NOT to say:
❌ "In general, you can..."
❌ "Typically, users would..."
❌ "This usually means..."
❌ "Try checking if..."
❌ "Most systems have..."

## What TO say:
✅ "According to [Document], you can..."
✅ "The User Guide states that users should..."
✅ "The documentation defines this as..."
✅ "Section X.Y mentions checking..."
✅ "Our system specifically has..."

Gem #2: The Description Arms Race

Your knowledge source descriptions are actually instructions in disguise. Use them:

Bad: "HR Policies"

Good: "HR policies document containing vacation, sick leave, and remote work policies. 
Does NOT include salary information, performance review process, or hiring procedures. 
Last updated: January 2024"

The agent uses these descriptions to decide if it should search that source. Be explicit about what’s NOT there.

Gem #3: Multi-Channel Testing Is Not Optional

Your agent acts different in different places:

Copilot Studio Test Chat: Dr. Jekyll - follows rules perfectly Teams: Starting to get creative Production Web Chat: Mr. Hyde - full hallucination mode

Why? Different context windows, different system prompts being injected, different auth contexts. Who knows? Just test everywhere.

Gem #4: The “Confidence Trap”

The more confident the agent sounds, the more you should worry. I added this to my prompt:

When sharing information, use appropriate uncertainty markers:
- "According to the documentation..." (when quoting)
- "The guide mentions..." (when paraphrasing)
- "I found that..." (when searching)

Never state things as absolute facts unless directly quoting.

Lesson 1: Default Settings Are Actively Working Against You

That “general knowledge” toggle? Should be OFF by default for any business use case. But it’s not. Because Microsoft apparently thinks we’re building creative writing assistants.

Lesson 2: Your Prompt Is Code, Not Suggestions

Treat your prompt like production code:

  • Version control it
  • Test it thoroughly
  • Have rollback plans
  • Document why you wrote it that way

That weird repetitive instruction that seems redundant? There’s probably a reason. Past you was smarter than current you thinks.

Lesson 3: Users Will Find Edge Cases You Never Imagined

Real questions I got:

  • “How do I sync with my Tesla?” (We make internal business tools)
  • “Can I use this on my PlayStation?” (It’s a web app)
  • “Does it support Sanskrit?” (We’re a US company)

Each of these made the agent try to be “helpful.” Don’t let it.

Lesson 4: “I Don’t Know” Is A Feature, Not A Bug

Train your users that “I cannot find this information” is GOOD. It means the agent is being honest. I even added this to our FAQ:

Q: Why does the agent keep saying it can’t find information?

A: This is by design. Our agent only provides information from verified documentation. If it says it can’t find something, that means either:

  1. The feature doesn’t exist
  2. It’s not documented yet
  3. You need human help

This prevents the agent from giving incorrect information.

Lesson 5: Hallucinations Are Like Weeds

You’ll never eliminate them completely. They’ll pop up in new and creative ways:

  • When you add new knowledge sources
  • When Microsoft updates their models
  • When users ask questions in unexpected ways
  • When Mercury is in retrograde (probably)

Stay vigilant. Keep testing.

When your prompt isn’t strong enough, stuff the context:

When all else fails, and you absolutely cannot have hallucinations, build a validation layer:

Is this overkill? Yes. Did I consider it after the third hallucination incident? Also yes.

The Bottom Line

Building a Copilot Studio agent that doesn’t hallucinate requires:

  1. Turning off general AI knowledge (seriously, do this first)
  2. Writing paranoid prompts that assume the worst
  3. Testing like you’re trying to break it (because users will)
  4. Accepting that “I don’t know” is the right answer sometimes
  5. Monitoring and updating constantly because AI gonna AI

Is it more work than the “5-minute agent” Microsoft demos show? Hell yes.

Will it save you from explaining to leadership why your agent told someone to delete System32? Also yes.

Choose wisely.


P.S. - I know some of you are thinking “just use RAG and vector embeddings and fine-tune the model.” Cool. I’m trying to get work done here, not write a PhD thesis.
